Immunization Requirements
Immunizations are one of the most effective and scientifically proven measures to protect children from harmful diseases and death. Students must have up-to-date certificates of immunizations at the time of registration for entry into kindergarten, 7th grade, and for enrollment into a new school district, grades 1-12th.
Parents must have one of the following on file for their child by Thursday, August 24 or the student may not attend school:
1. A valid, current immunization record.
2. A medical doctor’s (M.D./D.O.) signed State of Michigan 2023 Medical Contraindication Form, which states the medical contraindication(s), the vaccines involved, and the time during which the child is not able to get the vaccines.
3. A current, certified State of Michigan 2023 Nonmedical Immunization Waiver Form is required under Michigan Administrative Rules. Waiver forms must be obtained at the Shiawassee County Health Department. Please be aware that currently appointments for waivers are only scheduled on Tuesdays and immunization shots given at the Health Department can be scheduled on Wednesdays.
